Where can I get a leather holster made locally? (CZ P-09)

Locally to the Salt Lake City area, I'd recommend Eric at http://www.hbeleatherworks.com/

Eric made a custom holster for me using my pistol as the mold and it has held up very well over the years.

"Reasonable cost" is pretty subjective. The holster I got from Eric has cost me about $5/year and the cost has helped support a local craftsman. The holster you linked to is, I believe, made in another country and appears to be mediocre quality, at least as far as the stitching goes. The sample in the photo has double-rows of stitching that are not parallel to each other and the stitching is not parallel to the edge of the leather on the left side (in the photo) by the belt slot. Admittedly I haven't seen Eric's recent work (recent being within the last 9 years) but I would expect excellent quality from an established local holster maker no matter who it might be.

Which reminds me: I've seen good reviews of the holsters made by Nate at Ugly Bald Guy Holsters at http://www.ubgholsters.com/ in Stansbury Park, although I haven't personally seen any of his work.

Other than the custom pocket holster from HBE Leatherworks, all my IWB holsters are from Milt Sparks, the Versa Max 2 model.

I would expect to pay closer to $100 for a custom leather holster. You'll drop $75-$80 on a factory Galco leather holster easily, same goes for Desantis and several other brands that I've priced out. Hand made to fit is well worth the difference. If you really want to keep your cost below $80, consider a leather/kydex hybrid from a custom maker. I have an OWB from concealment solutions that's a great close fit, but it has no retention. Jason is now making a full coverage kydex holster on a leather background with retention for $85.00.
