We can accept 5 walk-ins, but you must arrive early, have the correct change, and be able to follow printed instructions for filling out the application and fingerprint cards!!!

Note: Whatever form of ID you bring, the name on the ID must match the name submitted to the course. If the ID and name submitted do not match, then we cannot give you a certified course completion certificate or completed fingerprint card.
If your driver license is your form of ID and the name and/or current address is incorrect, then we will accept the temporary ID form issued by the DMV indicating that you have submitted a correction to your name in the DMV licensing system. The Utah Department of Public Safety issues both the Utah Driver Licenses and the Concealed Firearm Permits, so the names and addresses MUST match to receive the CFP.
Please bring exact change $5.00 range fee and/or a $20.00 ammunition fee to the course, depending on which student category applies to you (see below).

$37.00 is due upon submission of the completed application package that you'll receive on the day of the training. You have 1 year to submit this package to the Utah Department of Public Safety, Bureau of Criminal Identification (BCI). If you choose to have me submit your package for you, then you'll need to have a check for $37.00 or credit card on the day of the training.

Registration will close on 10 November 2016. No registrations will be accepted after 10 Nov 2016.
Each student will have to pay a $5.00 range fee (required by the Lee Kay). Each student that uses one of my pistols and ammo will have to pay a $20.00 ammo fee.
Requirements:
1. Photo ID (driver license, military ID, federal or state government ID, passport, etc.). If you were born outside of the United States, then you must bring a photocopy of your Birth Abroad (US Citizens), Naturalization, or Permanent Resident ("Green Card").
2. Photo copy of ID.
3. Everyone must pay a $5.00 range fee. Please bring exact amount. Students using my handguns and ammo must bring an additional $20.00 to cover the cost of the ammo.
4. Must be able to possess a firearm (meeting Federal and State regulations). If you answered [Yes] to any of the questions on the registration and had a felony conviction as a result of the violation, then chances are that you are NOT allowed to possess a firearm or ammunition. PM me if in doubt.
5. Must be willing to stay for the entire duration of the class and range.
6. Encouraged to bring a handgun and 50 rounds of ammunition for the live fire range exercise. Note: Do not bring your handgun or ammo into the classroom. Leave it secured in your vehicle until the live fire exercise commences (1400 or 2:00 p.m., whichever comes first).
7. No drugs, alcohol, or contraband allowed.

A $5.00 range will be collected from everyone upon arrival at the range. Please bring the exact amount.
You will need to bring a handgun and 50 rounds of ammunition for your handgun. I have a few student 9 mm handguns. If you choose to use one of my student firearms, then you will need to bring an additional $20.00 to pay for 50 rounds of ammunition.
Lee Kay now open at 9:00 a.m. on Saturdays. I'll arrive early and attempt to get set up before 9:00 a.m.
